Part “B” Your Doctor must sign this form. If you are a FIRST year scout in Troop 45 this form will be required (Page 2) for Summer Camp 2010. 

Part “C” Everyone is required to complete this section. Only a parent signature is required. (Page 3). 

Page 4 – State of Michigan release form. Only a parent signature is required. This is the form to complete which allows another adult to remove your son from camp.
